Responsibilities of an RN Case Manager/Intake include personal care functions as a member of the nursing team under the supervision of the DON. The position of a Rn Case Manager/Intake will be a criteria-based position with well defined responsibility. Position responsibility shall be defined by but not limited to the following:
• Intake/Referrals taken complete referral sheet, obtain SOC orders.
• Case manages with doctor and insurance as needed
• QA/QI Charts 1-2 every week
• Phone calls/patient management
• Call hospitals and complete daily log
• Over see staff and assist DON/ADON with skilled and PAS departments
• Review weekly patient satisfaction surveys in skilled and PAS report to DON findings
• Review weekly infection control log and follow up in skilled and PAS report findings to DON
• Review weekly patient complaint/grievance log and follow up in skilled and PAS report finding to DON
• Review QA/QI 485 and give to DON for signature after completed and corrected
• Skilled nurse visit in building as needed
• Assist as needed answering phone take name and phone number, write message and relay
• Assist as needed in daily office functions
• Ensure the use of effective interpersonal communication skills, both verbal and non-verbal
• Ensures objective clinical observation, reporting and documentation of the client’s status
• Ensures accurate reading of vital signs
• Ensures compliance to basic infection control procedures and instruction on universal precautions
• Demonstrates knowledge of basic elements of body functioning and notes changes in body function that must be reported to the supervising R.N. DON
• Maintains a clean, safe and healthy care environment
• Recognizes emergencies and demonstrates knowledge of emergency procedures
• Promotes the physical, emotional and developmental needs of and the ways to work with the populations served by Alios including the need to respect the client and his/her privacy and property
• Attend and participate in orientation, training, educational activities and staff meetings when requested
• Maintains cost effective utilization of client supplies, soft goods and durable medical equipment.
• Treats clients with kindness, dignity and respect

• Assists in investigation and documentation of all complaints relevant to nursing practice and reports stated complaints to the Director of Nursing or Administrator

• Assists in development and implementation and maintains an ongoing evaluation relevant to clinical competence for Certified Nursing Assistants, Certified Medical Assistants and Home Health Aides

This will include, but is not limited to: Plan of care and treatment, Physician Orders, Discharge Summary, Case conference reports, Client service agreements and medication profiles
• Ensures that documentation in the medical record reflects care provided, client response, educational interventions, and support for financial reimbursement for services/care provided
• Ensures compliance with Infection Control Plan
• Establishes, implements, and maintains direct working relationships with physicians relevant to direct care issues
• Assists in development of new market development to maximize optimal patient care
